Claude Code tells me edits are complete but they're not

Anyone else seeing this? Claude says "I've updated the file" but when I check, the changes are partial or wrong. Started happening about 2 weeks ago. Running on macOS with the latest version.

SwiftMaker952: Factory reset fixed it temporarily but it came back after 2 days.

ZenMaker756: Have you tried resetting to factory settings? That fixed it for me.

DigitalHawk9933: PSA: downgrading to version 3.2 fixes this. Not ideal but works.
