Is it just me or has Claude Code gotten significantly worse at following instructions?

Two months ago Claude Code was incredible. I'd give it a task, it would do exactly what I asked. Now? It second-guesses everything. I say 'add a loading spinner to this component' and it rewrites the entire component, changes the styling, adds error handling I didn't ask for. Then when I say 'only do what I asked' it apologizes profusely and still does too much. The instruction-following regression is real.
